T These issues may last for several months and even years, something patients should be ready for if they decide to have the surgery. However, a lot depends on the patients age and the kind of surgery done. Studies show that about 60% of patients would make an almost complete recovery within 3 months with no long-term complications. By 12-months, almost 90% of patients can expect to feel much better and nearly normal. Unfortunately, improvement is slow for some people, and they need 2-3 years to revive their sexual function and get rid of urinary issues. Regretfully, these issues may continue to haunt some for a while. Here it is vital to understand that the above data is for radical proctectomy . The outlook is better after partial proctectomy. Are There Any Surgical Techniques That Have Been Developed To Improve Erectile Function OutcomesAt this time, there are several different surgical approaches to carry out the surgery, including retropubic or perineal approaches as well as laparoscopic procedures with freehand or robotic instrumentation. The Importance Of Prostate MassageProstate massage is a treatment that addresses the root cause of most prostate issues. It is a fact that modern medicine has not been able to determine the causes and treatments for many conditions related to the prostate. Impotence After Prostate SurgeryIn the past, up to 70 per cent of men who had their whole prostate removed because of cancer had some difficulty achieving an erection afterwards. This is because the prostate lies next to the nerves and blood vessels that are important for erections, and these nerves and vessels can be damaged during the operation. Newer surgical techniques that aim to spare the nerves associated with erectile function have reduced the risk of impotence. While there is a still a significant risk of erectile dysfunction after prostate surgery, there is often a gradual improvement in erectile function over time. Some men only have short-lived erectile dysfunction and others continue to improve for up to 3 years. It has been performed for many years and was regarded as the gold standard of prostate cancer treatment. However, few studies compare its efficacy to other techniques. Most men diagnosed with prostate cancer today are typically diagnosed with Gleason 6 cancer levels. But, according to many experts, this diagnosis may not be cancer! According to Mark Scholz, MD, a board-certified oncologist and expert on prostate cancer: Misuse of the term cancer has tragic implications. Real cancer requires action and aggressive medical intervention with the goal of saving a life. But consider the potential havoc created by telling someone they have cancer when it is untrue. This dreadful calamity is occurring to 100,000 men every year in the United States with men who undergo a needle biopsy and are told they have prostate cancer with a grade of Gleason 6. The impact of this is quite profound. Most prostate cancer diagnosed today falls into this Gleason 6. If it is not cancer, thousands of men have had aggressive treatment for cancer they dont really have. Aggressive treatment, usually a complete surgical removal of the prostate , is the typical result. This leaves the patient to suffer from its side effects for the rest of their life. Transurethral Resection Of The Prostate
Managing the Side Effects of Prostate Cancer and its Treatment
Transurethral resection of the prostate is a urological operation that involves cutting away a section of the prostate. It is most often used to relieve symptoms of urinary blockage, not necessarily to treat prostate cancer. An instrument called a resectoscope is inserted through the opening of the urethra and the surgeon removes the inner part of the prostate gland . The intervention lasts about an hour and it is most often used for non-cancerous blockage, but may also be used in cases of prostate cancer. The doctor doesnt need to make any incisions on the body. While most people can go home the same day, it is also likely that patients spend one or two days in the hospital. After the surgery, a urinary catheter will be placed because of the swelling that blocks urine flow. A new study published on Jan. 30 in the New England Journal of Medicine shows that while there were some differences in urinary, bowel and sexual function in the short term following prostate cancer treatment, there were no significant differences in side effects that were often present 15 years after treatment. About 238,590 new cases of prostate cancer will be diagnosed in 2013, and 29,720 men will die from the disease this year, according to the American Cancer Society. There has been recent debate between experts who say that prostate cancer should be routinely screened for and medical groups that argue men will suffer more and may not live longer if they treat the disease instead of leaving it alone. The U.S. Preventive Services Task Force, a non-federal panel of medical experts that advise government treatment guidelines, caused controversy in 2012 when it recommended that all healthy men should not be screened for prostate cancer because the PSA test used is not that accurate, and may pick up cancers too-slow growing to cause a problem. Meanwhile, the panel said prostate cancer treatments can carry risky side effects. Erectile dysfunction was reported in 87 percent of the prostatectomy group, while 93.8 percent of the radiotherapy group experienced those problems. Why Is It Important To Know The Risk Level Of Your CancerKnowing whether your cancer is low-risk, medium-risk, or high-risk is important when you are making treatment choices. The risk level of your cancer is based on the results from your tests and exams, such as the PSA test, digital rectal exam, and prostate biopsy.

A number of non-cancerous conditions, such as testicle injury or inflammation, can cause symptoms a lot like those of testicular cancer. Inflammation of the testicle and inflammation of the epididymis can cause swelling and pain of the testicle. Both of these also can be caused by viral or bacterial infections. Some men with testicular cancer have no symptoms at all, and their cancer is found during medical testing for other conditions. For instance, sometimes imaging tests done to find the cause of infertility can uncover a small testicular cancer. But if you have any of these signs or symptoms, see your doctor right away. More common than testicular cancer is epididymitis, which is inflammation of the epididymis, a tubular structure next to the testicle where sperm mature. About 600,000 men get it each year, most commonly between ages 19 and 35. Unprotected sex or having multiple sex partners increases the risk of infectious epididymitis. As many as one out of every five men has varicocele, which refers to swollen and dilated veins above the testicles , a condition that is usually benign. Testicular cancer Testicular cancer is one of the lesser common cancers but around 2,200 men in the UK are still diagnosed with it every year – incidence rates are increasing throughout the world and have more than doubled in the UK since the mid-1970s. The good news, however, is that survival rates for testicular cancer have actually risen year on year and the UK now boasts a 95% cure rate. Testicular cancer usually affects younger men between the ages of 15 and 49, and so if you are within this age bracket and/or experiencing any of the following signs and symptoms, it is important to visit your GP as soon as possible. The most common symptom is a lump or swelling in one of your testicles. Low Dose Rate BrachytherapyIn another type of radiation therapy termed low dose rate brachytherapy, radioactive pellets about the size of a grain of rice are inserted into the prostate. High Dose Rate BrachytherapyHigh dose rate brachytherapy applies more radioactive sources temporarily into the cancerous prostate gland. Both methods have side effects that can include erectile dysfunction, urinary tract problems, diarrhea, and other side effects. How Soon Can We Detect ThisOne of the main advantages of surgery over radiotherapy for prostate cancer is that following prostate removal, the PSA should be very low , which we can of course detect with blood tests. If metastasis occurs, because the metastatic cells originated in the prostate and therefore make PSA, the PSA level in the blood starts to rise. Once it has reached a given threshold additional or salvage treatment will be discussed. A PSA level of more than 0.2 ng/ml defines biochemical recurrence. At this stage the cancer is still much too small to be seen on scanning. If it can be seen on a scan it is termed clinical recurrence, which generally does not occur until the PSA level is more than 0.5 ng/ml. Symptoms, such as bone pain, dont usually occur until the PSA is more than 20 ng/ml. Fifty percent of men who are over the age of 60, and 95% of men over the age of 85, suffer from an enlarged prostate gland or benign prostatic hyperplasia . BPH typically occurs as a man agesâthe prostate gland can enlarge and compress the urethra, obstructing the flow of urine. In men, urine flows from the bladder through to the urethra. BPH blocks the flow of urine through the urethra. The prostate cells will slowly begin to multiply, creating an enlargement that places pressure on the urethra . The bladder needs to use more force to push urine through the body as the urethra narrows. Eventually, the bladder muscle will become stronger and thicker, yet much more sensitive, resulting in the need to urinate frequently. Prostate Health And The MicrobiomeFor a long time, scientists believed that the stomach was a sterile environment due to its gastric acid that melts everything in its path. However, in the early 1980s, they discovered the gut microbiome, the group of bacteria that survive gastric acid and influence our physical body and our mind. There is growing evidence that the gut microbiome has a role to play in the development and treatment of a slew of conditions, including prostate cancer. Nonetheless, there still needs a lot more research in order to document the relationship between microbiome and the prostate gland. Be that as it may, a healthy gut microbiome can promote a healthy prostate gland. The following are some great tips on how to keep gut bacteria happy:

Discussion with your doctor or nurseAt each appointment, your doctor or nurse will ask how youve been since your last appointment. Tell them about any symptoms or treatment side effects youve had, as well as any other problems or concerns. You can tell them how you are feeling emotionally as well as physically. You can also discuss any practical problems you might have, such as problems at work or with day-to-day activities. You may be given a questionnaire about your physical, social, emotional and practical needs. You might hear this called a holistic needs assessment form. Your GP or hospital doctor or nurse can help you deal with side effects, or refer you to someone else who can. For example, if you have problems with leaking urine , they might refer you to a continence service. Or if you have problems getting or keeping erections , they can refer you to an erectile dysfunction service. They can also help you get support for emotional problems, such as feeling anxious or depressed, and practical problems, such as managing your finances. You might feel embarrassed talking about some of the side effects of treatments, such as erection problems. But remember doctors and nurses see people with these problems every day, so be as open as you can. They are there to help. PSA testYou may be asked to avoid any vigorous exercise or ejaculating in the 48 hours before a PSA test, as this could cause a temporary rise in your PSA level. Am I Too Old To Have Prostate SurgeryHow old is too old to have a radical prostatectomy?William D. DeWolf, M.D., Chief of the Division of Urology at Beth Israel Deaconess Medical Center, says: For surgery to make sense that is, for the benefits to outweigh the risks you need to be young enough to take advantage of the added survival time that surgery can offer. Thats why I generally dont recommend this surgery for a man whose life expectancy is less than 10 years, or for a man who is older than 75, depending on his personal and family health history. Radical prostatectomy is a major operation that can cause serious and life-threatening complications such as blood clots in the legs and lungs, abnormal heart rhythms, heart attack, pneumonia, and infections. The risk increases in older men and in those with other medical conditions, such as heart and lung disease, high blood pressure, diabetes, or a history of blood clots. Some studies have shown that side effects, namely urinary incontinence and erectile dysfunction, also increase with age. Prostate Surgery: What To Expect On The Day Of SurgeryThere are a few prostate surgery types and techniques, and what you can expect from your procedure depends on which one your surgeon uses. An incision may or may not be required all, some, or none of prostate gland may be removed and some surrounding tissue may or may not be excised as welljust to name a few differences. Life without a prostate can be close to normal for some, though not free from the after effects of prostate removal. There are few prostate surgery complications. Some are acute, and others chronic. Among acute side effects could be a reaction to anesthesia, bleeding issues, blood clots in legs, damage to nearby organs, and infection at the surgery site. Success Of Radical ProstatectomyThe goal of radical prostatectomy is to cure prostate cancer. However, prostate cancer cure is only possible from prostatectomy if prostate cancer is limited to the prostate. During radical prostatectomy, the removed prostate is examined under a microscope to see if prostate cancer has reached the edge of the prostate. If so, the prostate cancer has probably spread. In these cases, further treatments may be needed. Men with no evidence of prostate cancer spread have an 85% chance of surviving 10 years after radical prostatectomy.Ã Also Check: Can You Beat Stage 4 Prostate Cancer How The Surgery Is DoneDifferent surgical methods may be used to remove the prostate:

Recovery from cancer is not always possible. If the cancer cannot be cured or controlled, the disease may be called advanced or terminal. This diagnosis is stressful, and for some people, advanced cancer may be difficult to discuss. However, it is important to have open and honest conversations with your health care team to express your feelings, preferences, and concerns. The health care team has special skills, experience, and knowledge to support patients and their families and is there to help. Making sure a person is physically comfortable, free from pain, and emotionally supported is extremely important. People who have advanced cancer and who are expected to live less than 6 months may want to consider hospice care. Hospice care is designed to provide the best possible quality of life for people who are near the end of life. You and your family are encouraged to talk with the health care team about hospice care options, which include hospice care at home, a special hospice center, or other health care locations. Approximately 6-8 percent of men who have had surgery to remove their prostate will develop urinary incontinence. The good news is that most men will eventually regain bladder control with time. You May Like: What To Do For Prostate Cancer Surgery For Prostate CancerSurgery is a common choice to try to cure prostate cancer if it is not thought to have spread outside the prostate gland. The main type of surgery for prostate cancer is a radical prostatectomy. In this operation, the surgeon removes the entire prostate gland plus some of the tissue around it, including the seminal vesicles. Why Do Doctors Perform ProstatectomiesSince the prostate gland is not an organ essential for survivability, doctors recommend its removal in some cases. Perhaps the number one cause of removal of the prostate gland would be early-stage prostate cancer. In such cases, doctors may go for complete prostate removal, or they may even go for radical proctectomy, in which they also remove the surrounding tissues. However, prostate cancer is not the only cause. Doctors may also recommend prostatectomy to individuals with high risk or high probability of prostate cancer. Such individuals would often have some other severe symptoms like issues urinating. Doctors may also remove the prostate gland if there are issues like acute urinary retention, recurrent urinary infections, recurrent hematuria, bladder stone, bladder outlet problems not responsive to conservative treatment, renal insufficiency due to chronic bladder obstruction, etc. So, prostate cancer may be the leading cause of prostatectomy, but there are many other reasons why doctors may recommend it. Moreover, when people live with different non-cancerous prostate issues, they are also at an increased risk of developing prostate cancer. Read Also: Foods To.avoid After Prostate Surgery When You Meet With Patients Who Have Opted For Prostate Cancer Surgery What Complications Seem To Cause The Most AnxietyPatients are usually concerned about longer-term quality-of-life issues, such as urinary control also called urinary continence and changes in sexual function. Prostate cancer surgery does sometimes have a negative impact on these functions, although the likelihood depends on a variety of factors, including age, the extent of the cancer, and baseline function, or how well everything worked before the procedure. The outcomes for urinary continence at MSK are what I would consider to be excellent. More than 90 percent of our patients will regain urinary control, although they may go through a period perhaps several months or a year after surgery in which they do not have complete control. For sexual function, the extent of recovery is especially affected by the nature of the cancer. Unfortunately, the nerve tissue that allows a man to get an erection is right up against the prostate. We obviously want to remove all the disease, and if the cancer extends outside the prostate at all, its not wise for us to try to preserve the nerve tissue because we might leave some cancer behind. You often see claims made by institutions or surgeons that the patients they treat recover their erectile function in 90 percent of cases. Thats true only for a very select group of patients, usually those who are younger and had full erections prior to surgery. Urinary Incontinence After Prostate SurgeryUrinary incontinence and/or inability to urinate are the common side effects after prostate surgery. This side effect usually ends in a short time. Transient urinary incontinence develops in consequence of disruption or discomfort in the sphincter muscles which control the release of urine. This type of incontinence is generally similar to the stress incontinence that women often experience after vaginal delivery. After prostate cancer surgery, the goal is basically to recover quickly, have full control over the bladder and become sexually active again. Recommended Reading: Do Over The Counter Prostate Meds Work Factors That Increase Risk Of RecurrenceThere are many different factors that can help you determine what your risk of recurrence is. Your doctor will go over these during the initial course of treatment, therefore stressing the importance of follow up appointments. One factor is the involvement of your lymph nodes. If your cancer has metastasized to your lymph nodes, recurrence is more likely. Similarly, the larger the tumor, the more likely that you are to experience complications and rates of recurrence. If the tumor is intertwined or growing into other areas of your body, this also increases risk. The Gleason score is a system of grading your prostate cancer based on severity and localization. The higher the grade, the more likely youll experience recurrence. Finally, the stage of prostate cancer affects recurrence rates. When caught early, recurrence is not as likely as cancers that are in stage three or four. Cancer can be extremely aggressive and will change your life forever. Make sure that youre doing everything you can to avoid potential problems. Eat well, exercise regularly, and get enough sleep. Strengthening your immune system is a great way to reduce your risk of recurrence. Im Worried About All The Side Effects From Prostate Cancer Medications What Can I Do

After surgery: If youve had a radical prostatectomy, radiation therapy might be an option, sometimes along with hormone therapy. After radiation therapy: If your first treatment was radiation, treatment options might include cryotherapy or radical prostatectomy, but when these treatments are done after radiation, they carry a higher risk for side effects such as incontinence. Having radiation therapy again is usually not an option because of the increased potential for serious side effects, although in some cases brachytherapy may be an option as a second treatment after external radiation. Sometimes it might not be clear exactly where the remaining cancer is in the body. If the only sign of cancer recurrence is a rising PSA level , another option for some men might be active surveillance instead of active treatment. Prostate cancer often grows slowly, so even if it does come back, it might not cause problems for many years, at which time further treatment could then be considered. Factors such as how quickly the PSA is going up and the original Gleason score of the cancer can help predict how soon the cancer might show up in distant parts of the body and cause problems. If the PSA is going up very quickly, some doctors might recommend that you start treatment even before the cancer can be seen on tests or causes symptoms. Can A Man Have A Normal Sex Life After Prostate SurgeryThe question is loaded what is a normal sex life anyway? Most men will experience a slowing down or drop off in their sexual activity as they get older. That said, it is still a source of pleasure and intimacy amongst older couples, and for those for whom it is an important part of their relationship, they are keen to understand what sex after surgery will look and feel like. The majority of men with a diagnose of localized prostate cancer will undergo a conservative approach known as active surveillance, but some of them will require surgery at some point. The surgeons will excise the growth, or in some cases remove the entire prostate. There are also less invasive surgeries, such as the TURPs and other non-surgical therapies such as radiation therapy. With careful intervention, most men will be able to resume a healthy sex life after the initial recovery period. If you have had a radical prostatectomy without a nerve-sparing technique, theres a high chance you will experience a significant reduction in your sexual sensation and libido Also Check: Stage 4 Prostate Cancer Survivor Stories source https://www.healthyprostateclub.com/what-happens-when-you-have-prostate-surgery/ Via https://healthyprostateclub.tumblr.com/post/705850796192006144 Natural Treatment Options For Prostate Pain

Three Stages Of An Enlarged ProstateIt is difficult to tell how many men actually have an enlarged prostate since most men do not have any noticeable symptoms. It is believed there are three stages. The first is known as the microscopic stage where small areas of the prostate are enlarged but can only be seen through a microscope. Symptoms are rare at this stage, and only one in four men will ever develop symptoms. The macroscopic stage shows larger areas under a microscope, and can be felt by Dr. Steven Gange during a rectal exam. The symptomatic stage is noted by changes in urinary habits. There is decreased blood flow to the prostate and there can be additional symptoms related to inflammation of the prostate, an infection, or even prostate cancer. > > > All Natural Technique Fixes Enlarged Prostate Watch Here< <Surgical procedures to remove the diseased prostate are usually necessary. In advanced prostate cancer, cancer cells spread beyond the prostate to other parts of the body. These cells usually spread to the bones first, and doctors refer to this as bone metastasis. If prostate cancer spreads to the bones, it most often reaches the spine, ribs, and hips. This occurs in stage 4 prostate cancer, and it can cause pain. According to ZERO, an advocacy group, bone metastases will affect more than 60 percent of men with advanced prostate cancer. However, if a patient has more advanced prostate cancer, we have to do additional tests to ensure theres no involvement of the cancer in the bone. A doctor who suspects or already has diagnosed prostate cancer will look for characteristic changes in the bone. The X-ray or CT scan also can indicate how much of your spine is affected and where. The ACS names 5-alpha reductase inhibitors as a class of medications that may help lower your prostate cancer risk. They work by blocking an enzyme in the body that creates dihydrotestosterone, which is the main hormone that causes prostate growth. That said, more research is needed to fully understand whether these drugs can significantly reduce prostate cancer risk in the long term, the ACS says. Consider A Daily Baby AspirinSpeaking of inflammation as a potential risk factor for prostate cancer, its possible that some anti-inflammatory medications could be protective, too, says Dr. Ross. This could include taking a baby aspirin a daybut theres a caveat. There are risks to this, so the American Medical Association hasnt recommended it, he adds. Those risks include the possibility of spontaneous bleedingso this definitely isnt a step you want to take without consulting your doctor first, he says. Don’t Miss: How Large Can The Prostate Grow Screening Diagnostic And Prognostic TestsMany men with slightly or moderately high PSA levels do not have cancer detected in their biopsy or do not have an aggressive form that needs treatment . Several new tests are being developed to help men with moderate PSA level elevations decide whether to have a biopsy . A composite test called 4Kscore uses measurements of four different proteins in the blood along with clinical information about the patient to predict the likelihood that the cancer will spread within the next 1520 years . A meta-analysis of published data concluded that the diagnostic accuracy of this test is similar to that of the FDA-approved Prostate Health Index . Genetic markers from cells in urine collected after a digital rectal exam may be useful predictors of cancer aggressiveness . In a trial with 1,077 participants, combined measurement of the gene markers TMPRSS2:ERG and PCA3 improved the ability to identify men at low risk for aggressive disease compared with PSA testing alone . Researchers examining genetic markers in urine samples from 905 participants with prostate cancer found two other markers that could help identify those with high-grade cancer . These genetic tests could someday play a role in avoiding unnecessary biopsies . New blood tests provide patients and doctors with information to better select appropriate treatment strategies . Immunotherapy, which has been a huge advance for cancer care in general, is currently an active and promising area of prostate cancer research . Causes And Risk FactorsThe risk of prostate cancer, like many other forms of cancer, increases with age. Forty-year-old men have a 2.3% chance of being diagnosed with prostate cancer by age 60. For 70-year-old men, the 20-year risk of prostate cancer is 9.5% . African-American men have a higher risk of prostate cancer than Caucasian men . In one study of almost three million servicemen, African Americans were nearly three times more likely to be diagnosed with prostate cancer . According to the Centers for Disease Control and Prevention , about twice as many African Americans die from prostate cancer as Caucasians . In contrast, Asian Americans have a lower risk of prostate cancer . Men with a family history of prostate cancer and those who have specific gene mutations have an increased risk of prostate cancer . For instance, mutations in the BRCA1 or BRCA2 genes increase prostate cancer risk . The likelihood of developing prostate cancer before age 65 is more than eight-fold higher in men with mutations in the BRCA2 gene. The presence of BRCA2 gene mutations predict a lifetime prostate cancer risk of 3040% and is associated with more aggressive forms of cancer. While BRCA1 mutations have also been correlated with increased prostate cancer risk, the relationship is not as strong as with BRCA2 mutations . Gonorrhea is associated with increased prostate cancer risk . Through a case-control study, comparing blood samples from 834 men diagnosed with prostate cancer with those from 1,393 men not diagnosed with the disease, the study found that there was an increased risk of prostate cancer among men with high blood concentrations of Omega 3 fats, resulting in the following:
But dont panic! It is also important to know that this is just one study, and there are several other studies that suggest fish oil is actually linked to a lower risk of prostate cancer. Although the results of this study may have you throwing away your fish oil capsules, it should be noted that there are several inconsistencies. Exercise And Lose WeightExercising and losing weight are some of the best things you can do to promote prostate health. Many studies show that moderate or vigorous exercise reduces risk of BPH and urinary tract symptoms and helps with prostatitis as well. Exercise also benefits your cardiovascular health and can help prevent other health and sexual problems such as erectile dysfunction. If you are overweight, losing weight is one of the most important natural changes you can make in improving your prostate health, whether your concerns are cancer, BPH or prostatitis. A study published in the Journal of Urology found that overweight men, especially men with a high amount of abdominal fat, have an increased risk of BPH. If you are looking to shrink the prostate, losing weight can help you reduce your prostate size and help relieve annoying and frustrating urinary symptoms. On top of that, weight loss can reduce your risk for prostate cancer and help relieve prostatitis, too. Selenium: Questions And Answers
Early results of SELECT reported in 2009 found no difference in the rate of prostate cancer among the 4 groups. In the selenium alone group, there was a slight increase in the rate of diabetes mellitus. Even though this change was not clearly shown to be due to the supplement, the men in the study were advised to stop taking the study supplements. In 2014, further results of SELECT showed that selenium supplements in men with low selenium levels at the start of the trial had no effect on prostate cancer risk. However, selenium supplements in men who had high levels of selenium at the start of the trial increased the risk of aggressive prostate cancer. A study of 1,434 men in SELECT suggested that changes in certain genes which control the way selenium is used by the body may have an effect on the risk of prostate cancer. Several factors may have affected study results, including the dose of vitamin E and the form of selenium used. The authors concluded that men should avoid selenium at doses that are higher than the recommended dietary intake. Also Check: How To Check Your Prostate Do Eggs Raise Prostate Cancer Risk24 October, 2011 Eating just three eggs a week increases chance of men getting prostate cancer, reported the Daily Mail. The story went on to say: Experts in the US claimed that men who consume more than two-and-a-half eggs on a weekly basis were up to 81% more likely to be killed by the disease. This research examined the association between eating red meat, poultry and eggs and the risk of developing lethal prostate . The study was in a large group of 27,607 healthy men, of whom 199 developed lethal prostate cancer over 14 years of follow-up. The researchers calculated that the men who ate the most eggs were at a significantly higher risk than those who ate fewer eggs. No significant association was found with any other food item. This large cohort study has some strengths, such as its large size and the fact that information on the participants diet was continually updated over the course of the study. However it also has several limitations, and only a small number of lethal cancers actually occurred, which could suggest that this association is due to chance. Furthermore, these results are inconsistent with previous research, which found no significant association between eggs and prostate cancer. The findings will need to be confirmed in more robust studies before any firm conclusions can be drawn. Read Also: Best Food For Prostate Problems Disclaimer And Safety Information

Claim: Coconut Water Is A Virtual Fountain Of Youth And Stops AgingSort Of: Although its true that coconut water is packed with plant hormones called cytokinins it might be a bit far out there to say that it stops aging. The cytokinins in coconut water do support cell division and encourage rapid growth but they wont put aging on hold. It can help to slow the effects of aging, however. Research has suggested that when people consume diets rich in cytokinins, they have a lower risk of developing age related disease and degenerative issues. Coconut water is the richest natural source of cytokinins, so drinking it certainly wont hurt but it wont stop your next birthday from coming. PSA After Radiation | Ask a Prostate Expert, Mark Scholz, MD
Kiran Panesar, BPharmS , MRPharmS, RPh, CPhConsultant Pharmacist and Freelance Medical WriterOrlando, Florida US Pharm ABSTRACT:Anemia is a common complication inpatients with cancer. It can be caused by either the tumor itself or thechemotherapeutic regimen used. The severity and prevalence of thecondition varies based on a number of factors. Treatment is aimed atincreasing the oxygen-carrying capacity of the blood, reducing fatigue, and improving the patients overall quality of life.Erythropoiesis-stimulating agents, iron supplementation, and red bloodcell transfusions have all been recommended in different settings. Whilethese have proven beneficial in some patients, the optimal therapeuticmodality is yet to be established. Recent Update In The Pathogenesis And Treatment Of Chemotherapy And Cancer Induced AnemiaCancer and chemotherapy-induced anemia is relatively common but underdiagnosed.
During Watchful Waiting Or Active SurveillanceIf you choose observation or active surveillance, your PSA level will be monitored closely to help decide if the cancer is growing and if treatment should be considered. Your doctor will watch your PSA level and how quickly it is rising. Not all doctors agree on exactly what PSA level might require further action . Again, talk to your doctor so you understand what change in your PSA might be considered cause for concern. Don’t Miss: Us Task Force Prostate Cancer Screening Following Psa Levels During And After Prostate Cancer TreatmentA mans prostate-specific antigen blood level is often a good indicator of how effective treatment is or has been. Generally speaking, your PSA level should get very low after treatment. But PSA results arent always reliable, and sometimes doctors arent sure what they mean. Before starting treatment, you might want to ask your doctor what your PSA level is expected to be during and after treatment, and what levels might cause concern. Its important to know that the PSA level is only one part of the overall picture. Other factors can also play a role in determining if cancer is still there, if it is growing, or if it has come back. Its also important to know that PSA levels can sometimes fluctuate a bit on their own, even during or after treatment, so they may not always be a sign of what is actually happening with your cancer. Understandably, many men being treated for prostate cancer are very concerned about even very small changes in their PSA levels. The PSA level is an important tool to monitor the cancer, but not every rise in PSA means that the cancer is growing and requires treatment right away. To help limit unnecessary anxiety, be sure you understand what change in your PSA level might concern your doctor. Salvage Radiotherapy And Androgen Deprivation TherapyGETUG-AFU 16 was the first randomized trial comparing SRT vs. SRT and short ADT as salvage treatment for biochemical recurrent prostate cancer after radical prostatectomy and was presented in abstract form at the American Association of Clinical Oncology 2015 Annual Meeting. The trial randomized 743 patients most of them having high intermediate risk features . The 5-year PFS was 62.1% vs. 79.6% for SRT and SRT+ADT, respectively . The 5-year OS was 94.8% for RT vs. 96.2% for SRT+ADT . Cause of death was progressive disease in 2.1% of the patients on SRT arm vs. 0.8% in the SRT+ADT arm. Acute toxicities occurred more frequently in SRT+ADT arm . This trial will require longer follow-up to see if the benefits observed in progression-free survival translate into the same OS benefit . Don’t Miss: Can Prostate Cancer Be Cured Radiation Therapy: What It IsThis therapy, also known as radiotherapy, is a cancer treatment procedure that uses high doses of radiation to kill cancerous cells and shrink the tumor as well. At low doses, this procedure is used as an x-ray. This therapy can be internal or external or both form. For external beam, a machine that is outside your body aims at the cancerous cells. For internal therapy, the radiations are placed inside your body inside or near the cancer. For radiotherapy for prostate cancer, high-energy rays are used to kill the cancer cells. This treatment procedure does not cause pain. However, it may result in various side effects that might cause pain and make you feel uncomfortable. The good thing is that there are numerous ways to manage radiotherapy side effects with the help of your radiation oncologist. Types Of Aplastic AnemiaThere are two types of aplastic anemia:
You May Like: Prognosis For Metastatic Prostate Cancer How Is Anemia DiagnosedA diagnosis of anemia can be made through a complete blood count laboratory test that measures your hemoglobin level and other characteristics of your red blood cells, including their size. To determine what is causing the anemia, a number of other tests can be performed, including blood chemistry tests, a bone marrow exam, and test of your stool to check for internal bleeding. Another test that can diagnose iron-deficiency anemia is serum ferritin level. Ferritin is a protein that stores iron. Low levels of ferritin lead to iron-deficiency anemia. Elevated levels of ferritin, on the other hand, are often found in patients with conditions that cause the body to absorb too much iron, such as hemochromatosis. Elevated levels of ferritin can also be due to inflammation. High levels of ferritin have been found in patients with some forms of cancer, such as lymphoma, cervical, and breast cancer, and high serum ferritin levels are associated with poor survival in some cancers. Prevalence Of Anemia In Radiation OncologyRadiation oncologists often “inherit” anemia that has developed from surgical blood loss, myelotoxic chemotherapy, and/or advanced disease. The prevalence of anemia among patients presenting for radiation therapy is not well documented, but it is generally believed that a substantial proportion of these patients do become anemic. A recent literature review revealed a relatively high incidence of mild-to-moderate anemia in patients receiving single-agent or combination chemotherapy for nonmyeloid malignancies. A similar assessment in the radiation oncology setting is not available. An ongoing retrospective study at the Beth Israel Medical Center is assessing the prevalence of anemia immediately prior to and during radiation therapy. We are performing this study through random chart sampling of patients who had received radiation therapy since December 1996. As of June 1999, a total of 574 patients were evaluable, with a relatively even distribution of cancers of the prostate , breast , head/neck , colon/rectum , lung/bronchus , and uterine cervix . The overall prevalence of anemia at presentation for radiation therapy was approximately 41% . At completion of radiation therapy, 43% of men and 63% of women had anemia , which usually was of mild-to-moderate severity . Worsening Anemia Signals Poorer Outcomes In Men Treated For Advanced Prostate CancerResearchers from the Oregon Health & Science University Cancer Institute and Southwest Oncology Group have identified a new method of determining how men with advanced prostate cancer will respond to treatment. They found that worsening anemia during the first three months of hormonal therapy for prostate cancer that has spread predicts shorter survival and earlier relapse. These results suggest that by monitoring anemia during the first three months of treatment, we can provide men with a better idea of how well they will fare, said principal investigator Tomasz Beer, M.D., director of the prostate cancer research program in the OHSU Cancer Institute. Beer presented results of this study at the 101st Annual Meeting of the American Urological Association in Atlanta on Tues., May 23. Researchers also found that race alone was not a strong predictor of survival or disease progression. However, they found that men with the same hemoglobin levels before treatment experienced significantly different overall and progression-free survival depending on whether they were black or white. Hemoglobin levels in the blood are measured to monitor anemia. Lower levels of hemoglobin are considered anemic. Overall, researchers found that anemic blacks fare worse than anemic whites and that blacks with high baseline hemoglobin fare better than whites with similar hemoglobin levels. This study was funded by the National Cancer Institute. The good news is that anemia caused by your chemotherapy is treatable. Your doctor may give you medications to treat your anemia. Your doctor may also order a blood transfusion for your anemia. A blood transfusion is when you are given red blood cells donated by another person. The blood that you are given is tested to make sure that it is healthy. When you have a blood transfusion a bag of blood is given to you through an I.V. . A blood transfusion will help your body until it can make more red blood cells. Overview: Radiation Therapy & Prostate Cancer | Michael Steinberg, MD | 2021 PCRI Con
Radiation therapy uses concentrated doses of radiation to kill cancer cells and reduce the size of tumors. Depending on the type of cancer present in the body, one of two types of radiation therapy may be used. External beam radiation therapy uses a large machine to send radiation into the specific area containing cancer. The radiation machine never touches the body, but it does move around to deliver radiation into precise parts of the body. External beam radiation is the most common type of treatment for many cancers. Internal radiation therapy, on the other hand, uses a solid or liquid radiation source to physically deliver radiation inside the body. If a solid source of radiation is used, it only targets a specific part of the body for localized treatment, especially for cancers of the head, neck, breast, cervix, prostate, and eye. If a liquid source of radiation is used, its considered a systemic therapy that travels through the blood into tissues throughout the entire body. Radiation therapy is often used in conjunction with other treatments or surgeries to target cancer in the most strategic way possible. Its often used to make surgery easier by shrinking the size of the tumor beforehand. Radiation therapy is even used during surgery to go straight into cancer cells without passing through the skin. Recommended Reading: Multiple Myeloma And Prostate Cancer What Are The Types Of Radiation Therapy Used For Prostate CancerRadiationtherapy for prostate cancer can be divided into two main categories. Externalbeam radiation :Using a machine outside the body, beams of radiation are focused on theprostate gland. This can help relieve symptoms such as pain while limiting thedamage to the tissues surrounding the prostate. The 4methods of external beam radiation are:
Brachytherapy:Small radioactive pellets are inserted into the prostate, each one about thesize of a grain of rice. About 100 pellets are used to limit the damage tosurrounding tissues and organs. Brachytherapy is most often used forearly-stage cases and is sometimes combined with EBRT. The pellets can eitherbe inserted for a couple of days for high dosages or a few months for lowdosage depending on the patients overall status. Radiotherapy Effects On Your BloodRadiotherapy sometimes slows down the cells in the bone marrow that make your blood cells. This is more likely if you’re having treatment to a large area of your body. Or having treatment to your: Having chemotherapy with radiotherapy can make the effects on the bone marrow worse. You have blood tests before your chemotherapy to check your blood count. Unlike x-rays, which release energy both before and after they hit their target, protons cause little damage to tissues they pass through and release their energy only after traveling a certain distance. This means that proton beam radiation can, in theory, deliver more radiation to the prostate while doing less damage to nearby normal tissues. Proton beam radiation can be aimed with techniques similar to 3D-CRT and IMRT. Although in theory proton beam therapy might be more effective than using x-rays, so far studies have not shown if this is true. Right now, proton beam therapy is not widely available. The machines needed to make protons are very expensive, and they arent available in many centers in the United States. Proton beam radiation might not be covered by all insurance companies at this time. Urinary And Bowel ChangesRadiation therapy can cause permanent urinary and bowel changes. Many people dont notice any changes or have any symptoms. However, some people have late side effects. Late side effects may be similar to the ones you had during treatment. Theres a very small chance you may develop other side effects. For example:
These side effects are rare. They may come and go over time or be persistent and chronic. Your healthcare team will help you manage them. Even if you dont develop any late side effects, remember that the tissues in your bladder and rectum were affected by your radiation therapy. Call your radiation oncologist if you:

Another type of prostate issue is chronic prostatitis, or chronic pelvic pain syndrome. This condition causes pain in the lower back and groin area, and may cause urinary retention. Symptoms include leaking and discomfort. In severe cases, a catheter may be required to relieve the symptoms. If the problem is unresponsive to other treatments, your doctor may suggest a surgical procedure. If these do not work, your symptoms could progress and become chronic. An acute bacterial infection can cause a burning sensation. Inflammation of the prostate can affect the bladder and result in discomfort and other symptoms. This is the most common urinary tract problem in men under 50, and the third most common in men over 65. The symptoms of acute bacterial prostatitis are similar to those of CPPS. Patients may experience a fever or chills as a result of the infection. Donât Miss: How Painful Is A Biopsy Of The Prostate What Is The Prostate GlandThe prostate gland is about the size of a walnut and surrounds the neck ofa man’s bladder and urethrathe tube that carries urine from the bladder. It’s partly muscular and partly glandular, with ducts opening into theprostatic portion of the urethra. It’s made up of three lobes, a centerlobe with one lobe on each side. Researchers don’t know all the functions of the prostate gland. However, the prostate gland plays an important role in both sexual and urinaryfunction. It’s common for the prostate gland to become enlarged as a manages, and it’s also likely for a man to encounter some type of prostateproblem in his lifetime. Many common problems that don’t require a radical prostatectomy areassociated with the prostate gland. These problems may occur in men of allages and include: Cancer of the prostate is a common and serious health concern. According tothe American Cancer Society, prostate cancer is the most common form ofcancer in men older than age 50, and the third leading cause of death fromcancer. There are different ways to achieve the goal of removing the prostate glandwhen there’s cancer. Methods of performing prostatectomy include: Don’t Miss: What Age To Get Prostate Checked Prostate Cancer Surgery Should Be Individualized To Each PatientNot every prostate cancer can be treated with a single-port approach or retzius-sparing surgery, but there are other choices. We have a large and expanding repertoire of treatment options that allow us to personalize treatment to each patients needs, says Dr. Sprenkle. The potential options include several different surgical approaches and robots, as well as the alternative of radiation therapy, which uses high-energy rays or particles to kill cancer cells, delivered in precisely targeted external beam treatments or an implanted radioactive seed. For small tumors, another approach is focal therapy, a term for noninvasive techniques for eliminating the tumors while leaving the prostate gland itself intact. “Patients come to my office concerned about the potential side effects of surgerythey are afraid they will become incontinent or have erectile dysfunction, says Isaac Kim, MD, PhD, MBA, professor and chair of the Department of Urology at Yale School of Medicine. The best candidates for single-port surgery are men whose cancer is classified as favorable intermediate risk, Dr. Sprenkle says. He wouldnt recommend it for men with unfavorable intermediate or high-risk cancer, because the approach doesnt provide easy access for pelvic lymph node dissection to assess whether the cancer has spread. Some simply favor other approaches, Dr. Sprenkle says. The Purpose Of Prostate SurgeryProstate cancer surgery, or radical prostatectomy, is a procedure conventional medicine praises for curing prostate cancer. The aim of surgery is to remove all of the cancer cells. It is usually done when the cancer is contained in the prostate and has not spread to the surrounding area. A prostatectomy may sometimes be done to treat locally advanced prostate cancer. But usually, this operation is not possible. This is because the surgeon would not be able to remove all the cancer cells that have spread outside the prostate. Other types of surgery, such as transurethral resection of the prostate , can help with the symptoms of locally advanced prostate cancer. This treatment is an option for curing prostate cancer that has not spread beyond the prostate or has not spread very far. Like any operation, this surgery carries some risks, such as urinary incontinence and erectile dysfunction. In extremely rare cases, problems arising after surgery can be fatal. It’s possible that prostate cancer can come back again after treatment. Your doctor should be able to explain the risk of your cancer coming back after treatment, based on things like your PSA level and the stage of your cancer. Studies have shown that radiotherapy after prostate removal surgery may increase the chances of a cure, although research is still being carried out into when it should be used after surgery. You may want to ask your doctors about storing a sperm sample before the operation so it can be used later for in vitro fertilisation . Caring For The IncisionYou will be able to take a shower the second day after your surgery. You may continue to have some discharge at the drain site for three to five days. Once you leave the hospital, the key words on caring for the drain site and incisions are clean and dry. Showering once a day and gently patting the area with a clean towel should be sufficient. Who Should Undergo Radical ProstatectomyMen younger than age 75 with limited prostate cancer who are expected to live at least 10 more years tend to get the most benefit from radical prostatectomy. Before performing radical prostatectomy, doctors first try to establish that the prostate cancer has not spread beyond the prostate. The statistical risk of spread can be determined from tables comparing the results of a biopsy and PSA levels. Further testing for spread, if needed, can include CT scans, bone scans, MRI scans, and ultrasound. If it appears that the prostate cancer has not spread, a surgeon may first offer other options besides surgery. These can include radiation therapy, hormone therapy, or simply observing the prostate cancer over time, since many prostate cancers grow slowly. Depending on how high the risk of cancer spread, pelvic lymph node dissection may be considered, as well. Small Cell Carcinoma of the Prostate Explained
Unlike other small cell neuroendocrine carcinomas and their non-small cell histologic counterparts, there is a surprising overlap in the spectrum of genomic alterations seen in t-SCNC and mCRPC-adenocarcinoma. Similar numbers of single nucleotide and copy number variants are observed across t-SCNC and mCRPC-adenocarcinoma specimens, and most of the common PC alterations are detected in similar frequencies across the two histologic subtypes. This genomic overlap supports a divergent model of clonal evolution, whereby t-SCNC arises through transdifferentiation of an mCRPC-adenocarcinoma subclone – rather than treatment selection of a pre-existing independent neuroendocrine subclone. The paucity of genomic differences between mCRPC-adenocarcinoma and t-SCNC also underscores the potential role of epigenetic reprogramming in mediating lineage plasticity and establishing tumor phenotype, discussed below. In the context of this broad genomic overlap, however, the few genomic differences that have been described across the two histologic subtypes provide important insights into the pathogenesis of t-SCNC. Gene Expression Profiling Of Scpc/lcnec Xenografts Reveals Upregulation Of Mitotic GenesUnsupervised hierarchal clustering of the raw expression profiles obtained from the MDA PCa 79, 117-9, 130, 144-13, 144-4, 146-10, 155-2, and 155-12 xenografts showed that replicates from the same xenograft clustered together, reflecting biologic identity, and that the xenografts were classified according to their morphology and AR expression . SCPC/LCNEC xenografts were collectively compared against the adenocarcinoma xenografts. We identified 140 probes expressed differently , corresponding to 104 unique RefSeq Transcript IDs . Gene Ontology analysis showed enrichment in mitosis-related biologic process subtrees, including âcell-cycle phaseâ , âmitotic cell cycleâ , and âcell divisionâ , among the 104 genes expressed differently between the groups . Genes in these subtrees included CDC25C , ANLN , AURKA , HELLS , UBE2C , PTTG1 , KPNA2 , TACC3 , PDCD6IP , HIST1H4C , and PCNA . A correlation between small cell or neuroendocrine carcinoma of the prostate and poor survival outcomes was revealed, despite treatment with the combination of the immunotherapy agent atezolizumab and chemotherapy, according to an institutional study conducted within the Mayo Clinics Division of Medical Oncology. The exploration of this combination in prostate cancer was influenced by the survival benefit observed with atezolizumab and chemotherapy in patients with extensive-stage small cell lung cancer . It is a common clinical practice to use regimens from the SCLC space to treat rare extrapulmonary small cell carcinoma, therefore oncologists at the Mayo Clinic sought to test the treatment strategy in their institution. Recent introduction of checkpoint inhibitor therapy for SCC of the prostate was based on extrapolation from studies in small-cell lung cancer. We wanted to review the outcomes for such patients who had received chemo-immunotherapy treatment at Mayo Clinic, the study lead Lance C. Pagliaro, MD, professor of oncology, Division of Medical Oncology, Department of Oncology at Mayo Clinic, told Targeted Oncology in an interview. Docetaxel with carboplatin is also a reasonable option for patients who have not previously received docetaxel, given the frequent mixed CRPC/NEPC features. A major clinical challenge is in what to do next after platinum-based chemotherapy. There are limited data in the second line and beyond settings. For mixed tumors with both adenocarcinoma and NEPC elements, the choice of therapy depends on the dominant histology and clinical context. Next line SCLC regimens may be considered for those with small cell NEPC these include lurbinectedin, topotecan, pembrolizumab, ipilimumab/nivolumab. Given limited data, we would encourage participation in clinical trials. Further, ADT should be considered in both de novo and treatment-related NEPC cases given tumor heterogeneity. For example, some dont cause your prostate specific antigen level to rise. This means theyre not always picked up by a PSA test. Because of this, some rare cancers may not be diagnosed until they have already spread outside the prostate. Read more about the PSA test and other tests used to diagnose prostate cancer. Some rare prostate cancers may only be picked up after having a biopsy to check for prostate cancer, or surgery called transurethral resection of the prostate to treat an enlarged prostate. The tissue removed during the biopsy or TURP is looked under a microscope to see if you have common prostate cancer or a rare type of prostate cancer. Rare cancers arent always given a Gleason score after a biopsy. This is because they can behave differently to common prostate cancer and cant be measured in the same way. Because rare cancer can be aggressive and spread outside the prostate, you will probably have more tests to see if they have spread. These include: Large Cell Prostate Cancer
Prostate Carcinoma Pathology ( Morphology and Gleason Grading)
Large cell prostate cancer is very rare. Because of this, we dont yet know how it develops, or the best ways to treat it. It is aggressive and can spread quickly to other parts of the body. Most men who have large cell prostate cancer also have common prostate cancer at the same time. And its most common in men whove already had hormone therapy for normal prostate cancer. Because neuroendocrine cells dont produce PSA, most men with large cell prostate cancer have a low PSA level. So a PSA test cant be used to diagnose or monitor your cancer. But if you have both large cell prostate cancer and common prostate cancer at the same time, you may have higher PSA levels. More research is needed before we can know whether PSA tests can help to diagnose large cell prostate cancer mixed with common cancer. Most large cell prostate cancers are diagnosed when they have grown large enough press against the urethra , which can cause difficulty urinating . So large cell prostate cancer is often diagnosed when men have surgery called transurethral resection of the prostate to treat their urinary problems. Tissue removed during surgery is looked at under the microscope to confirm you have large cell prostate cancer. You will also need scans to see if your cancer has spread. Most men with large cell prostate cancer have chemotherapy . You may also be offered surgery and radiotherapy, depending on how much the cancer has grown and spread.
